Cambridge Health Alliance Earns National Recognition for Patient-Centered Care


NCQA Patient-Centered Medical Home (PCMH) RecognitionCambridge, Mass. — Cambridge Health Alliance (CHA), the comprehensive academic community health system serving Cambridge, Somerville, and Boston’s metro-north communities, announced that three of its primary care practices have received the highest level of NCQA Patient-Centered Medical Home (PCMH) Recognition for using evidence-based, patient-centered processes that focus on highly coordinated care.

CHA’s Windsor Street Health Center, Somerville Campus Primary Care, and Cambridge Primary Care Center were all awarded PCMH Level 3 Recognition, the highest standard of patient-centered care. They join seven other CHA practices (Revere Family Health Center, Everett Family Health Center, Union Square Family Health, Cambridge Family Health, Cambridge Family Health North, Broadway Health Center, and the Malden Family Medicine Center) that have received recognition over the past five years.

“This is a tremendous accomplishment and a major milestone in our PCMH transformation,” said CHA Chief Executive Officer Patrick Wardell. “NCQA is widely considered to be the industry’s gold standard for health care quality improvement, and this recognition validates our efforts to advance the health and well-being of the community through team-based care, improvements in access, and reshaping how care is delivered to our patients.”

The NCQA Patient-Centered Medical Home is a model of primary care that combines teamwork and information technology to improve care, improve patients’ experience of care, and reduce costs. Medical homes foster ongoing partnerships between patients and their personal clinicians instead of approaching care as the sum of intermittent office visits.

CHA is deploying the model for all its ambulatory sites as part of a system-wide transformation to improve the health of its communities. The recognition is valid for three years.
