Care Dimensions

When patients have advanced and chronic health needs, palliative care is often the best treatment. Care Dimensions is CHA’s preferred partner to provide palliative care services to our patients.

What is Palliative Care?

Palliative care is a team-based approach to treating serious illness that focuses on the physical, emotional and spiritual needs. Palliative care is appropriate at any age and any stage of illness.

The goal is to prevent and relieve physical symptoms, anxiety and stress that often accompany a serious illness. This may include managing pain, shortness of breath, fatigue, loss of appetite and/or decreased function.

Benefits include:

  • Pain and symptom relief
  • Coordination of care
  • Patient and family support
  • Goals of care conversations
  • Help with decision making
